Where can I find the Condom Availability Program (CAP) Order Form?
Only staff who have completed the Condom Availability Program training may place an order.
For CAP certified Staff:
The CAP order form can be found in the CAP online community on Microsoft Teams.
To access the Microsoft online community:
- Open Microsoft Teams and sign in with your DOE credentials.
- From the left toolbar select Teams tab.
- Click on Condom Availability Program (CAP) Online Community
For non-CAP certified Staff:
Only staff who have completed the Condom Availability Program training may place an order. To learn more about the Condom Availability Program (CAP), please email cap2@schools.nyc.gov.
